Your privacy is of the upmost importance. All information relating to the provision of  child and family therapeutic services and supervised access is in accordance with the Privacy therapeutic services Act, 1988, and the Australian Association of Social Workers (AASW) Code of Ethics.  For further information is available on the AASW website. Client Information

Axia Therapy Services collects personal demographic information, including name, address, contact phone numbers and any other relevant medical or personal information that may be required to provide services.
All hard copies of client files and documents are securely held in a locked filing cabinet. And can only be accessed by authorised persons.

Collection of Personal Information.

Clients can provide relevant personal information via completion of forms, via email or contacting Axia Therapy Services directly to speak with a practitioner. Information may also be provided via referrals received from legal, medical or other health care practitioners.

It is important to note that the purpose of gathering personal information is to provide a service that best meets the needs of the child and family. At the initial appointment, a  biopsychosocial assessment is conducted. This assessment will help me gain a better understanding of the presenting problems from your perspective. The assessment also enables me to identify any biological and psychosocial contributing factors. This information helps me evaluate the best treatment options for your individual circumstances. Importantly, this information will ensure that all reports and documentation on sessions will be objective and factual.

Whilst all information provided is confidential, there are exceptions.

These exceptions include the following:

Information is subpoenaed by the court or disclosure is authorised or required by law.
Where there is a reasonable belief that not disclosing information may place a client or another person at risk of harm or risk to their life, health or safety.

Written client consent is required when a written report is to be provided to another agency or professional. For example, General Practitioner or solicitor.

Written client consent is also required  before I can discuss any matter with another person, such as a parent, another professional or a third party.

It is important to note, that as a client, you have the right to view and correct any personal information about you that is on file. Please speak with a practitioner at Axia Therapy Services who will be happy to discuss the content with you or if you so wish, provide you with a copy subject to exceptions in the Privacy Act 1988.  All requests will be responded to in writing within 30 days. If you are not satisfied with the response, please contact Tia Cherry, Axia Therapy Services on 0431291701 to make an appointment to discuss your concerns.

If you are still not satisfied with the response, you have the right to lodge a formal complaint with the Office of the Australian Information Commissioner by phone on 1300 363 992 or online: https://www.oaic.gov.au/privacy/privacy-complaints.

​Axia Therapy Services Cancellation Policy and Fees.

An SMS reminder will be sent 48 hours prior to your next scheduled appointment. Please provide 24 hours’ notice if you are unable to attend your appointment. In the case of non-attendance or where short notice is given, a cancellation fee of $100 will be incurred and must be paid prior to or at the next scheduled appointment. Please call 0431 291 701 if you wish to reschedule and appointment.

All session fees of $250 are payable at the end of each session. If you are unable to pay on the day, an invoice may be issued.

Please note that sessions are not eligible for a Medicare or Private Health rebates.

The fee for a court report is  $2200 inclusive of GST.

Full payment is required before the report can be released unless Legal Aid is responsible for costs. An invoice will be sent to you or your legal representative when the report is nearing completion.
