Axia Therapy Services values and respects the confidentiality and privacy of all clients and without your written permission, information relating to you will not be shared with any external party. The exception to this rule is:
- Information is shared with the referrer (GP, NDIS plan managers.
- If you report any suicidal ideation or thoughts of deliberate self -harm, harm to others
- If the information is requested by the court
- If there is concern regarding the safety and welfare of a child(ren) where, as a Mandated Notifier, there is a legal requirement to report concerns to Department of Child Protection.
- Subject to these exceptions Axia Therapy Services complies with the Privacy Act 1988. and other applicable privacy laws and regulations.
This Privacy Policy (Policy) describes how Axia therapy Services collects, holds, uses and discloses your personal information, and how your personal information is secured and maintained.

- Private Health Insurance details

- Emergency contacts
- Doctor/medical practitioner details
Why collecting personal information is important
Personal information and information collected from third parties such as GP referrals, NDIS, Psychologists or other mental health organisations both in the public and non-government sectors is important for the following reasons:
The information enables Axia Therapy Services:
- to gain a better understanding of the presenting issues contributing to poor mental health.
- To develop an individualised treatment plan to address the main mental health concerns
- To set treatment goals
- Develop a Relapse prevention Plan
How your personal information is protected:
- Only persons involved in your treatment will have access to the information.
- Axia therapy Services has instituted a number of technological systems such as anti-virus software and firewalls.
- Personal information is usually retained for the duration of treatment unless there are legal requirements to retain the information for a longer period of time.
Your rights
If you are not satisfied with any aspect of the service provided and are concerned about how your personal information is managed, you have the right to make a complaint, initially directly with Axia Therapy Services
